The Day of Your Appointment

If this is your very first appointment with us, please arrive 15 minutes before your scheduled appointment. This will allow us to finish any paperwork and stay on schedule with your appointment.
Please bring the following items with you to your first appointment:
- Dental Insurance Card (If applicable)
- Identification (Driver’s license, military ID or state ID)
- Patient Health History Form
- HIPAA/Financial Policy Form
- Privacy Practices
If you require pre-medication before your appointment, please be sure to request a prescription prior to your appointment, or if you are unsure, contact us and we can help. Please read the Pre-Medication Requirements Form.

Are my files transferable to your office?

Of course! If you are a current patient at another dental office you can either bring your files with you on your appointment or you can work with the current provider and ask them to send them to our office. If you have any questions feel free to call us.
